What if I am not happy with my support coordinator?
You can contact APD to request a new selection form.
How can ASCS help me?
Our fully trained support coordinators will schedule a meet and greet with you and your circle of supports to complete a psychosocial assessment, also known as a Person-Centered Support Plan, that will assist them with identifying your needs.
Do I have to pay for your support coordination services?
If you have been approved by APD, you will be assigned an Ibudget which will cover our costs.
